On-line Registration Form

A convienience fee equal to the fee PayPal charges CSQ will be added ($.30 + 2.9% of total). You are no longer required to be a PayPal member in order to use their sevices.

If you do not wish to register online, please print the form on the next page and mail it to the address listed below.

Refund of registration fee, less $10 for handling, will be made if cancellation is in the hands of the registrar in writing by January 10, 2014 at midnight.
